Mobile Medicine Outreach Medical Group links physicians to patients who may be homebound, disabled, frail or receiving hospice care. Our clients include primary care physicians, board and care facilities, and home health agencies.
Mobile Medicine Outreach offers complete medical management. Home based primary care program with the following services:
- Physician visits, treatment, medication, lab work and diagnostic testing performed in the comfort of the patient’s home.
- Utilization of the same medical equipment found in hospital emergency rooms and conventional physician offices.
- 24-hour on call physicians.
- Coordination of specialty consultations such as psychiatry, podiatry, radiology, physical therapy, DME, home health and hospice care.
- Comprehensive and timely progress medical reports to the referring care physicians.
- Medication compliance and prescription renewals.
- Pre-operative physical evaluations.

Founder and Medical Director, Jacob Flores, MD, established his company based on the principle that prompt, comprehensive medical care delivered in the home is far more desirable and less expensive than hospital-based options. The busy schedule of today’s private practice physicians, coupled with a sick patient’s lack of transportation and aversion to a hospital setting, fueled Dr. Flores’ belief that there is no better place to receive medical treatment than in the comfort and healing environment of one’s own home.
